Membership is a voluntary choice. Read the declaration, prepare your information and use the official application process. Submitting an application does not by itself confirm approval.

The official form covers your personal and contact details, electoral and local party placement, next of kin and signed declaration. Complete the applicant sections accurately and leave the official-use section for UDA.

Province & District
Geographic organisational placement.
Constituency & Ward
Electoral and local organisational placement.
Polling Station / Base Area
Local placement where applicable.
Registered Voter Status
Applicant voter-registration status.
Party Wing
Main, Women or Youth wing where applicable.
Declaration & Signature
Read the membership declaration, then provide the required applicant and witness details. The form includes next-of-kin information and a separate official review section.

Applicants declare that they are not, or are no longer, members of another political party; will abide by UDA’s constitution, policies, regulations and code of conduct; are or shall be enrolled on the Zimbabwe Electoral Commission voters’ roll; and have supplied true and correct information.
Keep personal information private
Use the official application channel for completed forms. Do not post identity documents, membership forms, next-of-kin details or contact information in public comments or public groups. Ask UDA how your information will be handled if you need clarification before applying.
